General Instructions for Award Nominations
A. Only one ESA member is needed to fill out a Nomination Form for any ESA award. Only an ESA member can fill out the form. It is the responsibility of the Nominator to obtain the necessary endorsements specified on the Nomination Form.
A.
1) Each form provides spaces for the Nominator to offer specific information about how the Nominee satisfies the criteria for each award. Additional information can be attached to the form by writing "See Attached" under the appropriate criteria. All information should be typed or clearly printed to insure legibility.2) Endorsements by people other than the Nominator are also required and vary according to the award. The Nominator can simply make a phone call, e-mail, etc. to obtain the required endorsements and then place each endorser's name on the appropriate line.
3) Finally, the Nominator's signature is required on the Form.
A. The Nominator is responsible for insuring the validity of the form, and the Nominator's signature attests that all the information is valid.
A. The Nominator can use e-mail to nominate by copying the Nomination Form from the ESA Home Page, filling in the information and sending the completed form to the Chair of the Awards Committee. (See e-mail address below.) The Nominator's e-mail address (which is transmitted with the e-mail information) will be considered equivalent to the Nominator's signature.
A. After the Nomination Form is filled out and signed by the Nominator, it should be sent to the Chair of the Awards Committee (TBA)
